    • 1). Complete a Wisconsin Withholding Exemption Certificate, Form WT-4. The Wisconsin Department of Revenue requires all new employees to complete this form. Use this form if you want to change your filing status, for exemptions such as claiming your dependents, or an exemption for yourself or spouse. Add additional income tax to be withheld from your paychecks or if you want to claim exempt from state income tax withholding.

    • 2). Make sure you have claimed only the exemptions that you actually have before signing the form and submitting it to your employer. When you sign the withholding certificate, you attest that you have claimed only the exemptions you are entitled to.

    • 3). Complete an Employee Withholding Agreement, Form WT-4A, if you want to request a lesser withholding amount. Use this form if you claimed the maximum amount of exemptions allowed on your WT-4 form, but you are paying more than you anticipate you will owe for Wisconsin income tax.
