    Obtaining a Replacement Social Security Card

    • 1). Visit SSA.gov and click on the link that says "Social Security Card" at the center of the page. Print and fill out form Form SS-5, which must be completed in order to obtain a new card.

    • 2). Find your local Social Security office by looking in the blue pages of your phone book. Social Security offices are in most towns, and replacement cards do not cost anything.

    • 3). Go to your local Social Security office with the Form SS-5, an official copy of your birth certificate and your driver's license, state-issued identification card or U.S. passport. If you are not in possession of one of these forms of identification, or you cannot obtain one within the next 10 days the Social Security office may request that you submit other documents proving your identity, such as an employee ID card, student ID card, health insurance card or a military ID card.

    • 4). Submit your application at the local Social Security office. Once it has been processed it usually takes about two weeks to receive your replacement card. Call the Social Security Administration at 1-800-772-1213 to inquire about the status of your replacement Social Security card.
