How to Change Your Marriage Name in Florida
- 1). Make copies of your marriage license. You'll need to give agencies a copy when changing your name. If you were married in a foreign country and the license is in a foreign language, you'll have to have a notarized translation.
- 2). File a form with the local Social Security office. There are almost 50 Social Security offices throughout Florida. Visit the Social Security website to find the one nearest to you. You'll need to fill out form SS-5, which you can download online or receive from the office. Show the clerk your marriage license and ID, and he'll process it for you. It will take a few weeks to receive your new Social Security card.
- 3). Change your driver's license. Visit the nearest department of motor vehicles office to update your Florida driver's license. You will not have to retake the test, but you will have to show your previous ID and marriage license and pay a fee.
- 4). Register to vote. You won't be able to vote if your ID doesn't match the name that the polling place has on file. Florida voters can register through the Florida Division of Elections by filling out the online form or by printing out the application form and filling out the requested information. You should check the box to specify that you are changing your name.
- 5). Change your name on all of your accounts. Undoubtedly, you have various accounts in your maiden name. This can include your car registration and insurance, homeowner's insurance, credit cards, bank accounts, library cards, frequent flier programs and other such items. Contact the company to change your information; it usually requires a letter of request and a copy of your marriage license.
